Nonsurgical phalloplasty (penile volume augmentation) is an outpatient procedure performed in a medical office under local anesthesia, typically completed in under two hours, from patient arrival to discharge

The ideal candidate for nonsurgical phalloplasty typically meets the conditions listed below; however, some patients may still qualify even if one or more criteria are not met. For more details, see the section “Special Populations.”


While the ideal candidate for nonsurgical phalloplasty is circumcised, some uncircumcised individuals may qualify based on their anatomy, expectations, and the type of dermal filler chosen. Uncircumcised patients are encouraged to undergo a personal evaluation to determine their suitability.

Special Populations

Uncircumcised patients seeking significant volume enhancements are advised to consider a “low, tight” circumcision at least three months before the procedure. A “low” circumcision means the scar is placed closer to the corona of the glans, ensuring optimal results and minimizing risks during the enhancement procedure.

While psychological support alone may not always resolve the issue, many patients experience significant emotional and psychological relief following an objective size increase through our phalloplasty procedures.

The average length of an erect penis is approximately 13 cm (5.25 in). A small penis is defined as one that measures less than this average but more than 7 cm (2.75 in) when erect. Individuals with a small penis are considered good candidates for nonsurgical phalloplasty.

A micro-penis, in contrast, is an abnormally small penis measuring less than 7 cm (2.75 in) during erection. Patients with a micro-penis are not suitable candidates for nonsurgical phalloplasty.

Micropenis and

Buried Penis

The buried penis (or hidden penis) is often associated with obesity or other conditions that enlarge the pubic fat pad. Despite being obscured, the penis itself may be of average size, making these patients good candidates for phalloplasty after liposuction of the pubic fat pad.

Uncircumcised patients with both a micro-penis and buried penis present the most challenging cases and are generally not ideal candidates for phalloplasty.

Prospective patients with previous procedures are assessed based on the results and nature of their prior treatments:

Previous Penile Procedures

Soft Silicone Scaffolding:

This procedure often results in irregularities but also causes a mild degree of fibrosis, which can facilitate the implantation of dermal fillers. These patients are generally good candidates for phalloplasty.

Autologous Fat Transfer:

Patients with silicone implants in place are not accepted due to a high risk of complications. If the silicone implant is removed through a secondary surgery, the patient can be reassessed, although they are considered less-than-ideal candidates for nonsurgical phalloplasty.

These devices, placed inside the corpora cavernosa, do not affect the superficial tissues and, therefore, do not disqualify patients. Such individuals can qualify for phalloplasty with dermal fillers

Rigid or Inflatable Implants:

These are implanted through invasive surgical procedures, including degloving of the penis. While reabsorbed by the body, they leave the skin firmer due to residual scar tissue. These patients are suitable for phalloplasty, though some areas may pose challenges due to surgical scars or adhesions.

Biological Membranes

(e.g., Alloderm™, Veloderm™)

Patients with a history of prior surgeries or implants must provide complete medical records and a summary of the current condition of the treated area. Failure to disclose prior procedures may result in disqualification; any expenses incurred will be the patient’s responsibility.

These are common because the implanted dermal filler “floats” within the skin of the penis. This differs from filler placement in the face, where it can be deposited on top of bone, and smaller volumes are typically used.

This complication is associated with the excessive use of dermal fillers in uncircumcised individuals. It occurs when too much filler is injected into the penile shaft, forming a thick layer under the excessively loose skin. When the foreskin and penile skin are pulled back to expose the glans, the loose tissue may bunch up at the base of the penis, creating a bulky, wrinkled appearance. In severe cases, this accumulation of thickened tissue can hinder penetration.
