Longevity is the defining property of PMMA-based penile girth enhancement. Nothing else in the field of soft-tissue augmentation lasts as long. This article summarizes what our clinic has observed in more than two decades of PMMA practice — what the tissue does at year one, year five, and year ten, and what patients should reasonably expect from a permanent result.
What PMMA Actually Is
Polymethylmethacrylate microspheres (30–50 μm) are non-degradable. Once placed in the subcutaneous plane of the penile shaft, they are encapsulated in the patient's own collagen. The result is not a foreign body sitting in tissue — it is a scaffold that becomes structurally continuous with the patient's connective tissue. This is why the outcome persists.

Year One: The Stabilization Phase
The first six months are dominated by tissue remodeling. Volume is visible immediately after treatment; edema resolves within two weeks; the true baseline result is established at approximately the three-month mark. Between month three and month six the tissue continues to organize around the microspheres. At the one-year follow-up the result should be indistinguishable from what was measured at month six.
Patients who evaluate their result too early are evaluating an incomplete process. Photographs at day three, day seven, or even week four do not represent the final outcome.
Years Two to Five: Where the Difference Shows
This is the window where PMMA separates itself from every other option. HA fillers have long since resorbed and required re-treatment; Ellansé has degraded past its peak. A well-placed PMMA result at year five looks and feels the same as it did at year one. Patients who elected multi-session staged plans typically complete their planned circumference by year two and then stop treating — the tissue holds.
No maintenance sessions. No re-injection cycles. No gradual loss requiring top-ups.
Years Six to Ten: What We Observe Clinically
The most common feedback at long-term follow-up is that patients forget they had the procedure. It becomes the anatomy. Circumference measurements at year ten in patients we have followed consistently match measurements at year one within measurement error.
Complications at this range have been rare when patient selection was appropriate. When they occur, they cluster around the predictable factors documented in the peer-reviewed literature: patients with subclinical autoimmune conditions, aggressive single-session augmentation performed elsewhere, or filler of unknown origin injected prior. What "Permanent" Actually Means
Permanent is not a marketing word here — it is a physical property of the material. Once the collagen capsule matures, the result is designed to last for the patient's lifetime. This is the greatest strength of PMMA and the reason it requires the most disciplined patient selection.

Frequently Asked Questions
How long does PMMA actually last?
Lifetime, by design. The microspheres do not degrade, and the surrounding collagen capsule is permanent tissue. Our longest-followed patients are past 20 years with stable results.
Can PMMA be removed if I change my mind?
Not easily. Unlike HA (dissolvable with hyaluronidase), PMMA is not enzymatically reversible. Surgical excision is technically possible in select cases but is not straightforward. This is why patient selection matters — PMMA is for patients who have already decided on permanence, ideally after first experiencing an HA result.
Will the result change as I age?
The PMMA scaffold and its collagen capsule do not change with age. What can change is the surrounding skin quality — as with any tissue. In our long-term follow-up we have not observed age-related shift in girth measurements.
What about weight loss or gain?
Significant weight changes affect subcutaneous tissue globally. In our clinical experience, moderate weight changes do not meaningfully affect PMMA girth results. Extreme weight loss or gain may produce visible changes in surrounding tissue but the augmented dimension remains stable.
How much girth increase should I expect over a full PMMA plan?
A single session typically produces 2.5–3.5 cm of mid-shaft circumference. Multi-session staged plans allow for larger cumulative increase over 2–4 sessions spaced 6–12 months apart. See our article on realistic before-and-after results for measurement context.
